Steps and Forms
In-patient
Here鈥檚 how you can avail of your membership benefits and get hold of the forms you鈥檒l be needing.
Step 2
Proceed to the adminitting section and have your 麻豆破解版 card swiped for validation of membership eligibility.
If APPROVED, the In-Patient Letter of Eligibility (LOE) will be printed.
Sign the LOE.
If DECLINED, attending medical staff will call 麻豆破解版鈥檚 Customer Service for assistance.
Step 3
Present the admitting order and LOE to the 麻豆破解版 Coordinator.
For emergency cases, confinements will be directly facilitated by the Emergency Room (ER) staff.
Step 4
Occupy allowed room.
Please double-check your room category/plan limit to avoid paying incremental charges.
Step 5
Inform 麻豆破解版鈥檚 Customer Service regarding admission within 24 hours.
Step 7
麻豆破解版鈥檚 Patient Relations Officer (PRO) will issue an 麻豆破解版 Referral Control Sheet (RCS) 3 for in-patient.
Sign the RCS.
Settle any incremental and miscellaneous charges upon discharge. Please validate if items charged are correct.
